Protected Health Information
GetPersonal by Converge is committed to protecting the privacy and security of patients’ health information in accordance with the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”) and applicable state laws. This Privacy Policy applies both to individually identifiable information that you provide to us for purposes of obtaining medical care through the Services (“protected health information” or “PHI”), which is subject to our HIPAA Notice of Privacy Practices, and information that is not PHI.
All patients receiving clinical services through GetPersonal are required to review and complete a HIPAA Notice of Privacy Practices and a HIPAA Authorization and Release of Information form as part of the intake process. These documents explain how protected health information (“PHI”) may be used and disclosed for treatment, payment, and healthcare operations, as well as patients’ rights with respect to their health information.
HIPAA-protected health information collected in connection with clinical services is governed by these HIPAA documents, which may provide additional protections and rights beyond those described in this Privacy Policy. This Privacy Policy supplements the HIPAA Notice of Privacy Practices for PHI. If there is ever any conflict between this Privacy Policy and the HIPAA Notice of Privacy Practices, the HIPAA Notice of Privacy Practices will apply. The HIPAA Notice of Privacy Practices does not apply to information that is not PHI.
Children’s Information
We do not knowingly collect personal information from, or market to, children under 18 years of age. If we learn we have collected or received personal data from a child under 18 years old without verification of parental consent, we will delete that information.
